Coronary Microcirculation

Coronary microcirculation forms a complex network of small vessels essential for supplying oxygen and nutrients to myocardial tissue. When dysfunction occurs—what’s termed coronary microvascular dysfunction (CMD)—patients can experience angina, ischemia, or even heart failure with preserved ejection fraction, all without the classic obstructive coronary artery disease visible on standard angiography. This makes CMD particularly elusive and challenging to diagnose.

To identify and assess CMD, clinicians rely on advanced technologies, such as coronary flow reserve measurements, positron emission tomography (PET), and cardiac magnetic resonance imaging (MRI), each offering insight into microvascular blood flow and myocardial perfusion.

Management focuses on optimizing endothelial health and improving microvascular function. Treatment strategies typically involve lifestyle modification, structured exercise programs, anti-anginal therapy, lipid-lowering agents, blood pressure management, and metabolic control. There is also emerging research into therapies addressing inflammation and endothelial dysfunction to further improve clinical outcomes.

Recognizing and treating coronary microcirculation disorders is crucial, especially for patients presenting with angina or ischemia despite angiographically normal coronary arteries. Early detection, targeted medical therapy, and comprehensive cardiovascular care are integral for symptom relief and prevention of disease progression, underscoring the growing importance of precision cardiology in this context.
